If you claimed a prize of more than $600 in an Iowa Lottery game during 2012, be on the look-out over the next few weeks for a form from the lottery that you'll need as you file your taxes.
The document is called a W-2G form. It lists the gross prize amount as well as any federal and state taxes that were withheld from that prize. If you claimed multiple prizes of more than $600 in calendar year 2012, you'll receive multiple W-2G forms from the lottery, one for each prize claimed.
As you can imagine, with the millions of winners that the lottery has each year, it takes some time to process all of those forms and get them ready to send out. The Iowa Lottery begins to review the records of its winners and prepare the documents as soon as each calendar year ends.
If you anticipate that you'll be getting a W-2G form from us and you've moved since claiming your prize, please let us know your new address as soon as possible. You can contact us at 515-725-7900 or email us at wmaster@ialottery.com. Be sure to include your name, old address, new address and phone number in case we need to contact you.
The W-2G forms must be mailed by Jan. 31 of the new year, but we try to have the forms processed before then if possible.
